Who can refer clients to our service?

We commonly receive referrals from:

  • General Practitioners (GPs)

  • Care Managers within Residential Aged Care or Ageing-in-Place Services

  • Support at Home coordinators

  • Lifestyle Coordinators or Managers within Independent Living Communities

  • Allied Health Professionals

  • Family Members

  • Self-referrals from individuals seeking proactive cognitive health support

Can the client use Home Care Package / Support at Home funding for this service?  
  • Clients may use their Home Care Package or Support at Home funding for Brain & Memory Clinic consultations, where the service aligns with their assessed care needs, goals, and provider approval

  • Our RN-led cognitive health education service can support clients with memory concerns, healthy ageing, independence, and care coordination

  • Care managers, package providers, and referrers are welcome to contact us to discuss suitability for a client’s care plan

Who are we?

Founded by Gemma, a Registered Nurse with over 20 years of aged care experience across Australia, Brain & Memory Clinic was created from a simple but powerful observation: many families wished they had received guidance earlier.

After years of supporting individuals and families affected by cognitive decline, Gemma became passionate about a more proactive, prevention-focused approach to brain health.

Using principles from the Bredesen Protocol® (ReCODE® framework), Brain & Memory Clinic provides clear, practical, education-based support focused on modifiable lifestyle and health factors linked to cognitive wellbeing helping clients take early, informed steps toward long-term brain health and healthy ageing.

Who is suitable

What costs are involved in the programs? Are there any additional costs?

✔ Cognitive Health Education Program – $195 AUD per visit at least 1 hour or over ($390 AUD total: one-time review program delivered over two visits)
A comprehensive education-focused review providing personalised brain health guidance and supportive lifestyle recommendations within nursing scope of practice

✔ Ongoing Cognitive Health Support Program – $149 AUD per month (minimum 6-month commitment)
An optional ongoing support program designed to provide continued education, progress monitoring, and personalised cognitive health support over time

✔ Optional client-initiated testing where appropriate
Additional biomarker or genetic testing may be discussed only where relevant to cognitive health education, care-plan goals, and lifestyle planning. Testing is arranged separately through accredited providers and may involve out-of-pocket costs unless ordered by a GP or specialist under Medicare

✔ Optional self-monitoring tools
Simple monitoring tools may be discussed where appropriate to support client awareness, routine, and healthy habit formation. These tools are optional, purchased separately, and are not required for participation
